Window tint medical exemptions in Hawaii
Hawaii does not provide one. Checked HRS 291-21.5 and Act 129 of 2025, the law that most recently amended it. Act 129 clarified the front side and wing window standard, exempted the rear window and the side windows behind the driver on sedans, raised the fines, and made a missing installer certificate prima facie evidence of non-compliance. It did not add a medical exemption. A separate 2025 bill, H.B. 368, would have added one as subsection (d)(12) for light sensitivity of the eyes, on documentation from a physician licensed under chapter 453, renewed every two years, where the condition could not be managed with eye protection. It did not pass; it was still at House draft stage and no session law enacting it exists. So the exemption Hawaii drivers are told about is a bill, not a law.
What applies instead
With no exemption in the statute, the ordinary limits stand: front side windows 35% VLT minimum, back side windows No limit specified, rear window No limit specified. A physician's letter does not change them, because there is no provision for an officer to apply.
The practical route for a sun-sensitivity condition is a clear or near-clear ceramic film. The good ones block 99% or more of UV and much of the infrared heat while metering well inside the legal VLT, so you get the protection without the darkness Hawaii forbids.

Does Hawaii have a window tint medical exemption?
No. It did not pass; it was still at House draft stage and no session law enacting it exists. So the exemption Hawaii drivers are told about is a bill, not a law.
Can a doctor's note make darker tint legal in Hawaii?
Not on its own. With no exemption in the statute, a physician's letter gives an officer no provision to apply, so the ordinary limits stand: front side windows 35% VLT, back sides no numeric limit, rear no numeric limit.
What can I do about sun sensitivity in Hawaii then?
A clear or near-clear ceramic film is the usual route: the good ones block 99% or more of UV and a large share of infrared heat at a VLT that stays inside Haw. Rev. Stat. § 291-21.5. That gets you the protection without the darkness the statute forbids.
