Window tint medical exemptions in Iowa
Iowa does not provide one. Iowa had a medical exemption and closed it. Iowa Administrative Code 761-450.7(2) provides that no exemption shall be granted from the minimum standard of transparency, effective 4 July 2012, and the standard is 70% light transmittance on the windshield, front side windows and front sidewings. The DOT states that form 432020 is no longer available, and that an exemption still in force would have been signed by the holder's physician before that date. Bills to restore one, including S.F. 48 (2023), have not passed.
What applies instead
With no exemption in the statute, the ordinary limits stand: front side windows 70% VLT minimum, back side windows No limit specified, rear window No limit specified. A physician's letter does not change them, because there is no provision for an officer to apply.
The practical route for a sun-sensitivity condition is a clear or near-clear ceramic film. The good ones block 99% or more of UV and much of the infrared heat while metering well inside the legal VLT, so you get the protection without the darkness Iowa forbids.

Does Iowa have a window tint medical exemption?
No. Iowa had a medical exemption and closed it. Can a doctor's note make darker tint legal in Iowa?
Not on its own. With no exemption in the statute, a physician's letter gives an officer no provision to apply, so the ordinary limits stand: front side windows 70% VLT, back sides no numeric limit, rear no numeric limit.
What can I do about sun sensitivity in Iowa then?
A clear or near-clear ceramic film is the usual route: the good ones block 99% or more of UV and a large share of infrared heat at a VLT that stays inside Iowa Code § 321.438; 761 IAC 450.7. That gets you the protection without the darkness the statute forbids.
